1. Requests for Statements of Discipline must be electronically filed, using the request form (located in the forms section of the website) in Miscellaneous Case Number 1:24-mc-527. In CM/ECF using the appropriate event “Statement of Discipline Request” which is located under Civil à Other Filings à Attorney Admissions Events. Please complete all fields in the filing event. The Statement of Discipline will be e-mailed to you.

2. You must include the following on your form request:

  • Full Name
  • Business Address
  • E-mail Address
  • Phone Number
  • SDNY Admission Date
  • SDNY or NYS Bar Number 

If you were admitted before October 6, 2018, your SDNY Bar ID is the initials of your first & last name followed by the last four digits of your social security number or the four numbers you selected at the time of your admission, e.g. Jane Doe1234 is JD1234.   

If you were admitted after October 6, 2018, your SDNY Bar ID is your NYS Bar number.

If you do not know your SDNY Bar ID please send an e-mail and include the following information to: Help_Desk@nysd.uscourts.gov 

  • Attorney's full name
  • Firm name
  • State bar number
  • Attorney’s e-mail address

If the email request is received during business hours, you should receive a response the same day.

3. Do not mail the form or deliver this form to the Court. It can only be filed on CM/ECF using your own individual Pacer account. Any mailings requesting Statements of Discipline will be returned to you. Attorneys who are admitted pro hac vice are not fully admitted to the court and are not eligible to receive a Statement of Discipline.

4. After you have made your Statement of Discipline request on CM/ECF, you will receive your Statement of Discipline via the e-mail we have on record for you. If you have not received your Statement of Discipline within two business days, please check your spam and/or junk e-mail to ensure the Statement of Discipline is not there. If you still cannot locate Statement of Discipline, please e-mail the Grievance committee with your name, the date you made your request, and your receipt number to: Grievance@nysd.uscourts.gov

Note: When making this request, please ensure that you go through the entire docketing event until the message appears that this entry has been the final docket entry on CM/ECF.
